Under the direction of the Community Services Department Manager, the Community Services Manager coordinates services, to access and participate in activities and functions of community life for individuals served. Works as part of a team to ensure person centered services. Reports on the progress of individuals receiving services, verbally and in writing to appropriate sources and will work with interdisciplinary team members in the development of the Individual Services Plan (ISP) and required documentation. Will assist with routine contact at community and home sites to ensure quality of service. Hires, evaluates, and terminates community specialist (as needed) in accordance with EEO and Affirmative Action guidelines with assistance from Community Services Department Manager, and Human Resource Director. Provides supports to individuals employed and participating in integrated settings throughout the Albuquerque area. Ensures Community Specialist is trained and familiar with each individual on their caseload to access their community as the individual desires.

Provide effective positive support and teaching strategies for people with Intellectual developmental disabilities who participate in the program. Provide personal care and monitor medications as needed. Attend Interdisciplinary Team (IDT), ISP meetings, staff meetings and trainings as requested. May be assigned to agency teams. Responds to and manages emergency situations appropriately. May include support outside of normal business hours for health and safety needs. Assist with the effective orientation training of new staff and substitute employees. Coordinates and schedules yearly and as needed individual specific training. Train Community Specialist on agency documents and reporting guidelines. Observe and report on individual dynamics, social/work-oriented roles and skills, behavioral interactions and other identified needs of individuals supported. Implement state standards for incident reporting consistently and accurately. May complete DVR process and billing. structure daily schedules, Ensure personal timelines are met demands of caseload in a timely manner. Complete required documentation Complete other projects as assigned by the Community Services Department Manager. Has limited access to Protected Health Information (PHI). Complies with HIPAA regulations and ARCA policies and procedures on confidentiality of information and records for persons served. Generally are responsible for light cleaning, cooking, running errands, as well as assisting clients with grooming, and other personal hygiene tasks. Ensures the success of individuals receiving services in attaining and maintaining vocational outcomes in integrated Work/Community settings. Encourages and develops natural supports, friendships, relationships in Community settings to ensure continued success by the person served. Must have the ability to encourage independence and fade supports. Provides employers and community sites with education and support services related to disability awareness, while maintaining the confidentiality of the person served. Participates with interdisciplinary team members in developing Person Centered plan to ensure individual success. Will be responsible to request necessary documents and ensure all documents are turned into the department Quality Coordinator timely. Evaluates and reports on the person’s performance and progress. Maintains accurate documentation and other related, required reports. Documentation will be completed in the time frame required by the Regulatory Standards. Creates and maintains positive relationships between individuals, families/guardians, nurses, and IDT members. Establishing and fostering community relationships. Which lead to meaningful community employment and memberships. Maintain a professional relationship with employers, individuals receiving services and family members at all times. Responsible for responding to maintain appropriate staff coverage for each individual assigned to caseload. To include hiring, supervising, individual specific trainings. Follow and comply with all guidelines and standards. May include Holidays and weekends based on the individual needs. Helps yearly conduct periodic evaluations of program, including satisfaction of consumers, families/guardians, employers and other interested parties. Responsible for the individual’s health and safety in regards to the service. Support access to medical or behavioral health services and assisting with medication delivery such as setting up or reminders. Ensure the Community Specialist has obtained appropriate health insurance. Develops and obtains community jobs for persons with Intellectual developmental disabilities through Relationships with employers and representatives to develop new opportunities for individuals served and to maintain professional relationships to ensure quality of service. Provide supports to implement the person’s purposeful and meaningful work, self-empowerment, personalize relationships, social, educational and community inclusion activities. Oversees individual’s financial benefits and personal financial accounts. Ensure that assets of the individuals do not exceed benefit limits by reviewing and managing individual’s collective accounts and working closely with ARCA’s finance department. Assist individuals with the management of their financial accounts. Meets monthly with each individual to review accounts make budgets and teach money management skills. Completes required documentation for SSDI, SSI, IRS, ISD, DD Waiver and other external agencies. Completes Section 8 housing applications and works with ARCA’s property department to ensure individuals

Schedule: Generally Monday through Friday, 9:00am-5:00pm, but flexible based on program needs. Weekends and evenings are required. On call every 3-4 Weeks.
