The Community Health Worker works in clinical, community-based and home settings to provide care coordination and connection to resources and support to patients to improve their health and general well-being through culturally appropriate education and coordinated care and services. Duties of this position include, but are not limited to health system navigation and resource coordination, completion of patient risk assessments and assisting patients with following through with care plans, medication access, and resources needed to achieve improved health outcomes.
This is part-time position up to 28 hours per week.
Qualifications and Skills:
A requirement of this position is to complete a South Dakota approved Community Health Worker certification program. Tuition and fees will be paid by the department.
Bi-lingual is preferred.
Positions at the Public Health Department are subject to the CMS Covid-19 vaccination requirements.
