Title: Continuum of Care Coordinator, Case Manager

Location: ASV Riverside Hospital

Department: Telemetry

Shift: Full-Time Days; 36hrs weekly

What You Will Do:
Provide coordination of patient care throughout the healthcare continuum.

Provide coordination of communications with patients, families, physicians, and healthcare team members.
Coordinate patient scheduling with appropriate resources or clinical services and maintain required documentation.
Support standard patient access initiatives across the healthcare continuum.
Provide support with navigating the healthcare system and assist patients with utilizing available community support services.

What You Will Need:
Licensure / Certification / Registration:
Registered Nurse credentialed from the Florida Board of Nursing preferred.
Social Worker credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.

Education:
High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
