Description:
The clerk position is an entry-level training role within the INVEST Program (Community Workforce Programs, HR). The position is full grant-funded and will not exceed 12 months in duration. Successful candidates will be placed at a Montefiore School Health Program (MSHP) site, and work under the supervision of a Site Administrator. Through the Invest program, the clerk will have access to career navigation services from the HR Community Workforce Programs team.

Responsibilities to include:
Engage parents of students via outreach in the community and school meetings.
Call homes of students to introduce the program and encourage enrollment
Work with school administration to support outreach efforts to parents, students and the community.
Encourage students to access regular health care.
Inform parents about weekly appointments for their children and engage them in the process to maintain their child’s health.
Help parents complete annual assessment and screening for their children to support providers.
Escort students in and out of classrooms for appointments.
Develop positive relationship with school administration to support MSHP.
Conduct social determinants of health screening with parents to assess family needs and connect them to services/follow-up and hare community resources and programs with school community.

Requirements/Qualifications:
High School or Associate’s Degree

Candidates must have successfully completed one of Montefiore’s Community Workforce Programs or partner programs.

Bilingual in Spanish, preferred
