The Hepatitis C Patient Navigator (HCV PN) will guide individuals thorough the Hepatitis C care continuum. The patient navigator will educate patients on the importance of screening, evaluation and treatment of hepatitis C. The patient navigator will assist patients with HCV along their care continuum, and help to address barriers that may exist during their evaluation and treatment. This may include outreach to help re-engage in care, assist with appointments and medication adherence, and linkage to community resources and/or specialty care.

Core Responsibilities

Develop a basic understanding of HCV screening, evaluation, and treatment and attend core HCV trainings at WFH.
Receive a caseload of Hepatitis C patients and make routine periodic contacts with all patients assigned. This will include previously diagnosed patients and newly diagnosed HCV patients.
Provide education, support and encouragement to Hepatitis C patients and help to address barriers to care and link them to additional WFH or community resources including: a Nurse case management, SW, MAT, behavioral health, PrEP
Counsel and educate patients on Hepatitis C by in-person, by telephone, or other electronic means and encourage them to be active participants in their own healthcare.
Support patients through the HCV care continuum during screening, evaluation, treatment and surveillance as needed. Provide direct reminders/phone contact to patients regarding: a. Appointments –including new diagnosis HCV appointment, follow-up appointments, and post-treatment appointments b. Lab work - bloodwork (baseline, on-treatment, post-treatment) c. Imaging and Procedures – including but not limited to elastography, ultrasound, and endoscopy d. Treatment – to assure medication adherence and need for refills, connect to nurse case manager as appropriate e. Ability to attend/accompany patients to appointments on an as-needed basis. Act as a source of support and ongoing education during the HCV care continuum.
Utilize i2i and CareMessage to assist with patient case management. Document in EMR as appropriate.
Work as team member within the patient’s treatment team, especially with the treating provider and nurse case manager.
Participate in community Hepatitis C events or programs such as HepAware

. Requirements:
Minimum Objective Qualifications

High school diploma or GED
Experience using computers and Microsoft Office software (Word, Excel and Outlook), and electronic medical record software

Preferred Objective Qualifications

Six months of experience in community services, counseling, teaching, healthcare or social services
College courses, Associate’s or Bachelor’s degree in education or a health-related field
Fluency in written and spoken English and spoken Spanish
