Job Title: Congregate Living Coordinator (CLC)
Department: Public Health Services
Reports To: Program Manager, Communicable Disease
FLSA Status: Non-exempt; Full-Time
Schedule: Monday-Friday 8am-5pm
Wage: $17.00-$27.00 hourly; depending on number of years of transferrable experience and internal equity

SUMMARY: Serve as a county-level liaison for congregate living facilities, including LTC/ALF, other congregate settings, and correctional facilities. In collaboration with Public Health Nurses, monitors and responds to outbreaks in congregate living facilities, including follow -up on reportable communicable diseases, education, data collection, training, and compliance. Collaborates with the MT DPHHS Infection Prevention Program for infection prevention strategies for seniors and long-term care. Participates in community health activities for congregate living facilitates. Work requires visits to long-term care facilities. This position reports to the Program Manager of Communicable Disease and does not supervise others.

ESSENTIAL KNOWLEDGE, SKILLS AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • *Practice
  • Monitor infections in congregate living facilities, including LTC/ALF, other congregate settings, and jails.
  • Develop and maintain partnerships with congregate living facilities for the purpose of identifying and responding to disease outbreaks.
  • Monitor and analyze trends in infectious communicable disease data and provide regular consultation to the congregate setting.
  • Understand, interpret, and communicate federal, state and local guidance.
  • Collaborate with the department’s epidemiologist, communicable disease and emergency preparedness programs in collecting data regarding health measures and conducting epidemiologic analysis needed to identify needs, prioritize public health interventions and strengthen active surveillance and response
  • Assess facility capacity to isolate and quarantine individuals with an infectious disease and their contacts.
  • Provide technical assistance to support patient care and isolation/quarantine recommendations.
  • Coordinate activities with the Communicable Disease team and other departments and outside agencies.
  • Attend meetings and special events

Communication

  • Conduct regular verbal and written communication with both congregate living facilities and Communicable Disease team about disease outbreaks, prevention and mitigation strategies.
  • Collaborate with congregate living professionals locally and across Montana to provide updates in a timely manner.
  • Develop and maintain relationships with congregate living facility partners.
  • Provide guidance, outreach and education and support the ongoing work in facilities to prevent and manage infectious diseases.
  • Ensure cultural and linguistic needs are addressed by public health for populations that are at higher risk for COVID and other infectious diseases in congregate living settings.
  • Ensure congregate living facilities are connected to county and state-level resources and support.
  • Collaborate with other organizations and agencies to provide comprehensive health services to individuals, and to develop population-based services.
  • Participate and facilitate committees as a public health professional and promote partnerships, collaboration and education associated effective public health services.

  • Proficiency using email and the Internet.
  • Ability to enter data accurately and generate reports from a data base
  • Participate in quality improvement initiatives.

  • Attend meetings and training sessions to become and remain current on practices and new developments in assigned scope of work.

  • *EDUCATION AND EXPERIENCE
  • A bachelor’s degree in a health related field of study OR
  • At least two years of experience in social services or a health related field OR
  • Any combination of education and experience which provides the equivalent scope of knowledge, skills and abilities necessary to perform the work.
  • Experience working with adults with developmental disabilities or a geriatric population preferred.

CERTIFICATES, LICENSES, REGISTRATIONS: Valid Montana Driver’s License in good standing.
