BACHELOR LEVEL - HOUSING OUTREACH TEAM (HOT) SPECIALIST

Position Summary: Bachelor-level candidate needed to fill a full-time, clinical position on a dynamic outreach team. Housing Outreach Specialists assist individuals in applying skills learned in therapy in the community. Specialists carry a caseload of approximately 15- 20 clients of whom they see one to many times per week. Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy. Basic fundamental computer experience to process e-mail and client access required.

Duties and Responsibilities:
Staff works with clients to accomplish tasks (such as grocery shopping, applying for benefits, securing housing, following through with medical treatment, etc.) through the successful use of skills (such as interpersonal, DBT, anger management, problem solving, and self-efficacy). This position requires staff to work closely with clinical case managers on a multidisciplinary team and requires critical thinking and strong organizational abilities. Staff must complete progress notes on all clients seen and attend treatment team meetings, supervisions, and group supervisions.

Knowledge, Skills and Abilities:
Experience working with people with severe mental illness is required.
Ability to work as an effective team player is essential.
Must be able to work independently and build own work schedule week to week.

Education: Position requires a bachelor’s degree in psychology, social work, rehab counseling or education/counseling, or related field from an accredited college or university.
