The Housing Stability area of Human Services and Public Health (HSPH) is seeking a Case Management Assistant who will become a part of a dedicated project team to help people experiencing homelessness and help ensure they do not return to homelessness.

Hennepin County has made the commitment to find and help secure permanent housing for people experiencing both sheltered and unsheltered homelessness within the Homeless to Housing Program. Case Management Assistants will be assigned to individuals in order to meet them where they are at, assess their housing barriers and goals and then join with them on the journey to secure and move-into safe and permanent housing.

Location and hours:
This position is hybrid and will work onsite at the Health Services Building at 525 Portland Ave S, Minneapolis, 55415 and remotely. This position may require travel to various shelters and drop-in centers throughout Hennepin County. Work hours are Monday through Friday, 8 a.m.- 4:30 p.m. Occasional evening hours may be required.

About the position type:
This is a full-time, benefit-earning position. You must be a current regular or probationary employee of Human Services and Public Health to apply for these positions.

In this position, you will:
Maintain a caseload of clients and actively work to permanently house them as a part of the Homeless to Housing Project; services will be provided in an ethical and culturally sensitive manner.
Work with people experiencing homelessness who may have mental health and/or substance use disorders.
Provide in-person outreach to people experiencing homelessness in shelters, drop-ins, and encampments.
Develop an initial housing plan with people to address where gaps in services exist in their support systems and refer to county or community agencies when appropriate.
If county resources are needed, collect information for eligibility review if needed to access these resources; help people complete the SMRT process and apply for Social Security Insurance.

Need to have:One of the following:
Two years or more of responsible paid social service experience providing direct client service.
A two-year human services certificate.
Note: College coursework in the behavioral sciences can be substituted for the required work experience.
A valid driver's license and access to reliable transportation for travel to various shelters and drop-in centers throughout Hennepin County.
Ability to use own vehicle on a frequent basis to transport clients to appointments and showings.
Work a flexible schedule including occasional evenings.

Nice to have:
Direct experience working with individuals facing homelessness.
Familiarity with databases and/or online tools such as Homeless Management Information System (HMIS) and Social Service Information System (SSIS).
Working knowledge of programs in Eligibility Works Services (EWS).
Work effectively with external and internal partners.
Communicate effectively both written and verbally.
