Description

Hennepin County's Office of Multi-Cultural Services within the Initial Contact and Assessment area of Health & Human Services is seeking a Case Management Assistant who is bilingual in both English and Oromo/Amharic, to be part of a team that provides Hennepin County residents access to information services, and supports their needs allowing them to live healthy, safe, stable, and independent lives.

Location and hours:
This position is hybrid and will be performed both on-site at the South Human Service Center at 2215 East Lake Street, Minneapolis MN 55407 and remote as job duties require. The work schedule will require staff to be flexible and to work virtually from home, on-site, in community settings, and at other various sites. Work hours will vary between 8 a.m. - 5 p.m., Monday through Friday, with occasional evening and weekend hours.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
Seek out, interview, and assist residents in need of agency services.
Conduct presentations before neighborhood groups, schools, healthcare providers, non-profits, Mutual Assistance Associations, etc. to develop and foster relationships between community resource providers and county departments by presenting information on programs, benefits, and resources.
Visit residents in their homes when necessary to explain supportive services and resources available to them.
Interact with schools, medical personnel, and community agencies to obtain information and learn about the services they provide.
Share information with county residents and assist them in making appointments.
Assist in resolving problems facing communities concerning health and social service issues including completion of forms for eligibility related issues, gathering verifications to assist in program eligibility determinations, connection to resources, etc.

Need to have:One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Ability to communicate fluently in Omoro/Amharic in addition to English, both verbally and in writing.
Access to reliable transportation to travel to service sites and residences within Hennepin County.

Nice to have:
Ability to translate to and from both English and Omoro/Amharic.
Experience working with the Oromo/Amharic-speaking community.
Experience using databases and performing data entry where accuracy is essential.
