Position Summary
Works at the Youth & Family Resource Center (YFRC) as an advocate for individuals and families with social service needs. Handles clientele complaint issues, requests, and demands accordingly.

Pay Grade: 51 Minimum Qualifications
100.00% - 1. Five (5) years of professional experience, which includes at least two (2) years in case management; AND

2. Associate"s degree from an accredited college of university in psychology, sociology, criminal justice, research and development or related field; OR

3. Three (3) years of social work experience, which includes at least two (2) years in case management and Bachelor"s degree (preferred) in psychology, sociology, criminal justice, research and development or related field; OR

4. An equivalent combination of education and/or experience is required.
5. PROOF OF EDUCATION, TRAINING AND/OR EXPERIENCE IS REQUIRED. Duties and Responsibilities
1. Conducts an intake assessment to collect information necessary to determine the participant"s specific needs.
2. Coordinates individualized plans with youth to meet short and long-term goals.
3. Provides service linkage and support to ensure success.
4. Maintains case files including legible, accurate and organized case notes/reports and provides follow up services as needed.
5. Motivates youth to participate in referred support programs.
6. Collaborates with referral service providers to locate other resources that will provide the best results possible for youths and their families, as needed.
7. Performs other related duties as required or directed. Knowledge, Skills and Abilities
KSAs - 1. Social work licensure is desirable.
2. Ability to work with at-risk youth.
3. Ability to effectively multitask to meet the demands of various cases simultaneously.
4. Communicates clearly to clients to understand each client"s circumstances and their options.
5. Excellent verbal and written communication skills including knowledge of business English and mathematics.
6. Demonstrate compassion, understanding, and empathy.

7. Strong organizational skills.

8. Ability to demonstrate understanding of models and theories regarding prevention and intervention for youth exposed to trauma.
9. Ability to locate efficient and effective wraparound service delivery for trauma-exposed youth
~ Ability to organize work, set priorities and monitor details.

10. Skill in utilizing word-processing and spreadsheet software.
11. Ability to organize and report information and develop presentations.
12. Ability to communicate effectively with others both verbally and in writing.
13. Ability to establish and maintain effective working relationships with co-workers, other County employees, service providers, outside organizations and the general public.

14. Ability to organize work, set priorities, and monitor details. Special Requirements
1. Must acquire CANS certification within 30 days of employment.
2. Traditional work hours are between 8:00 a.m. and 5:00 p.m., but a rotating or weekend schedule could be required, if needed.
3. Must be proficient in Microsoft Word, Excel and Outlook Disclaimer
